EMS FILE PROGRAM - FILE OF LIFE

The EMS File of Life card enables EMS Fire Department personnel to obtain vital medical information if the patient is incapacitated.
I can provide you with an EMS File of Life packet and help you fill out the medical information card.
This card, which is kept in the refrigerator in a yellow plastic container, includes patient name and emergency contact information, health problems, medications, dosages and special instructions, allergies, Doctor's name and phone number, and your insurance information.
A sticker for the front door identifies people with File of Life information inside. A second kit and sticker can be used for your car.
The File of Life provides a substantial preventative step that seniors can take. It's visibility and content ensure quick access to vital medical information.
